[I'm not quite sure about some of these chords, especially the Am7/G,
 which I play as 302013.  And the C might be C(9) (x32030).  Also, some
 of the other chords have non-root bass notes, like D/A or C/E, but I'm
 not going to try to figure out which ones.]
